A COPY OF YOUR RABIES VACCINATION MUST BE INCLUDED AND BE UP TO DATE!
**By licensing your pet within the Village of Alvo, you agree that if your animal is found to be running loose, you will accept a $25.00 fine on the 1st offense, a $50.00 fine for the 2nd offense and a $100 fine for the 3rd offense, per pet. If you receive a 3rd offense fine, you risk having your pet license revoked and possible removal of your pet from the Alvo Village limits.
You must be a responsible pet owner and follow the ordinances set forth with the Alvo Village limits that you keep your pet in the home, or if outside the home, on a leash, on a tie out or in a fenced yard at ALL times.
Under penalties of law, I declare that I have examined this application and that to the best of my knowledge and belief, it is true and correct and I am duly authorized to sign this application.
